Job Description

OhioHealth is seeking a compassionate Occupational Therapist to join our Healthcare & Medical Services team. In this role, you will assess, plan, and implement individualized treatment plans to help patients restore function, improve independence, and enhance quality of life across a variety of care settings. You'll collaborate closely with an interdisciplinary team, educate patients and families, and document progress in alignment with best practices. Responsibilities Conduct comprehensive occupational therapy evaluations, including functional, cognitive, and psychosocial assessments Develop and implement individualized, evidence-based plans of care focused on meaningful activities and functional outcomes Provide direct treatment interventions such as
ADL/IADL
retraining, therapeutic exercise, neuromuscular re-education, cognitive and perceptual training, and functional mobility Educate patients, families, and caregivers on home programs, adaptive strategies, and use of assistive devices or durable medical equipment Collaborate with physicians, nurses, rehabilitation therapists, case managers, and social workers to coordinate and optimize patient care Document assessments, treatment sessions, progress notes, and discharge summaries accurately and timely in the EMRParticipate in interdisciplinary care conferences and contribute to discharge planning and recommendations for ongoing services Monitor patient progress, reassess needs, and modify treatment plans and goals as clinically indicated Ensure compliance with regulatory, legal, and ethical standards, as well as Ohio Health policies and procedures Engage in continuing education, professional development, and quality improvement or evidence-based practice initiatives Provide mentoring or clinical instruction to OT students and onboarding support for new staff as appropriate Required Skills Occupational therapy evaluation and treatment planning ADL and IADL retraining Neuromuscular re-education Upper extremity rehabilitation and functional mobility training Cognitive and perceptual assessment and intervention Splinting and orthotic fabrication or fitting (basic level) Patient and family education and home exercise programming Interdisciplinary collaboration and care coordination Electronic medical record (EMR) documentation, preferably Epic Knowledge of rehabilitation best practices and outcome measures Understanding of insurance, authorization, and discharge planning processes Time management and caseload prioritization Cultural competence and trauma-informed, patient-centered care Professional written and verbal communication
