Compensation Package Competitive piece-rate pay with valuable incentives:
Per Visit Rate:
$70.00•$120.00 per completed visit (good-faith estimate; varies by visit type)
Mileage Reimbursement:
$0.72 per mile
Drive Time Pay:
$25.00 per hour
Documentation Bonus:
Reward for timely, accurate charting Job Summary The Occupational Therapist (OT) is responsible for implementing high-quality occupational therapy services in accordance with the patient's individualized plan of care and organizational standards. The OT works collaboratively with physicians, RNs, and the interdisciplinary team to support safe, functional recovery in the home setting. Key Responsibilities Provide patient, caregiver, and family education to support recovery and independence Communicate proactively with physicians and care team regarding patient progress and care plans Evaluate patient functional status, including strength, coordination, endurance, communication abilities, self-care skills, and work capacity Assess the home environment and identify barriers to safe, independent living Recommend and instruct on adaptive equipment, splints, prosthetics, and orthotics Develop individualized treatment plans and set measurable goals for improved function Teach or retrain patients in daily living skills impacted by illness, surgery, or injury Maintain accurate, timely, and compliant clinical documentation Job Requirements Valid California Occupational Therapist License Current CPR Certification 2+ years of clinical experience as an OT Reliable, insured vehicle and a valid driver's license Why Choose GrandCare Health Services?
