As a smaller, 47-bed skilled nursing facility, we pride ourselves on delivering personalized, high-quality care while fostering a supportive and collaborative work environment for our staff. What You'll Do Evaluate residents and develop individualized occupational therapy treatment plans. Provide skilled therapy services focused on maximizing independence and functional abilities. Collaborate with physicians, nursing staff, therapy team members, and families to achieve the best possible outcomes. Complete timely, accurate documentation in accordance with Medicare, Medicaid, and regulatory requirements. Participate in care conferences and interdisciplinary team meetings. Educate residents, families, and caregivers on adaptive techniques and equipment. Ensure compliance with all federal, state, and facility policies. Qualifications Current California Occupational Therapist license. Graduate of an accredited Occupational Therapy program. Current CPR/BLS certification (or ability to obtain). Skilled nursing or post-acute care experience preferred, but motivated new graduates are encouraged to apply. Excellent communication, organizational, and documentation skills. Passion for patient-centered care and teamwork.
