Job Description

Job Summary Smartsync are seeking a dedicated and energetic Occupational Therapist (OT) / Physical Therapist (PT) to join our dynamic healthcare team. In this role, you will play a vital part in enhancing patient outcomes through comprehensive occupational therapy services across diverse settings. Your expertise will support individuals of all ages, from pediatrics to geriatrics, helping them achieve their maximum functional independence. You will develop personalized treatment plans, collaborate with interdisciplinary teams, and implement evidence-based interventions to promote recovery and improve quality of life. Duties Conduct thorough occupational therapy assessments to evaluate patients' physical, cognitive, and emotional needs. Develop and implement customized treatment plans targeting specific goals such as improving ADLs (Activities of Daily Living), mobility, and functional independence. Assist patients with ADLs, including dressing, grooming, feeding, and other essential daily activities. Coordinate discharge planning by preparing comprehensive care plans and ensuring smooth transitions across care settings. Collaborate with speech therapists, physical therapists, physicians, educators, and other healthcare professionals to deliver integrated patient care. Manage electronic health records (EHR) accurately, documenting clinical notes, progress reports, and treatment outcomes in compliance with medical documentation standards. Support patients with neurodevelopmental disorders such as autism spectrum disorders by implementing specialized occupational therapy programs tailored to their needs. Provide occupational therapy services in various environments including inpatient units, outpatient clinics, skilled nursing facilities, home health settings, schools, and early intervention programs. Engage in case management activities to monitor patient progress and adjust treatment strategies accordingly. Educate patients and their families on therapy techniques and strategies for ongoing support at home or in community settings. Experience Proven experience working with individuals with disabilities across multiple settings such as inpatient care, outpatient clinics, schools, or home health environments. Demonstrated expertise in pediatric occupational therapy and early intervention programs supporting children with developmental delays or disabilities. Experience working with students with special education needs and implementing occupational therapy assessments within school systems. Strong background in acute care environments including ICU (Intensive Care Unit) and skilled nursing facilities with post-acute care experience preferred. Knowledge of physiology and anatomy relevant to occupational therapy interventions for diverse patient populations. Familiarity with medical terminology and clinical documentation standards necessary for accurate record keeping. Ability to develop effective treatment plans based on evidence-based practice principles tailored to individual patient needs. Skilled in client communication and clinical interprofessional collaboration to ensure holistic patient care. Experience working with individuals on the autism spectrum or neurodevelopmental disorders is highly valued.
