Responsibilities Perform occupational therapy evaluations and reassessments Develop individualized treatment plans Help patients improve safety, independence, and functional performance within their home environment Recommend adaptive equipment and environmental modifications Educate patients and caregivers Collaborate with physicians and interdisciplinary team members Complete documentation accurately and timely Qualifications Active California Occupational Therapist License Valid driver's license Home health experience preferred, but not required.
