Skilled Nursing Facility in San Leandro, CA Responsibilities Conduct occupational therapy evaluations and re-evaluations for residents in a skilled nursing facility Develop and implement individualized treatment plans to address activities of daily living, mobility, strength, and functional independence Provide direct therapy services, monitor progress, adjust plans of care, and document outcomes Collaborate with an interdisciplinary team (nursing, physical therapy, speech therapy, social work) to coordinate care and optimize discharge planning Educate residents and families on home exercise programs, safety, and adaptive strategies Maintain accurate, timely documentation in accordance with facility standards and regulatory requirements Requirements Availability for a 13-week travel assignment with guaranteed 35 hours per week Ability to work effectively in a skilled nursing facility environment with an interdisciplinary team Experience performing OT evaluations, developing treatment plans, delivering therapy, and documenting progress Strong communication and collaboration skills with residents, families, and care teammates Commitment to professional practice standards and facility policies Benefits and Support Guaranteed hours: 35 per week Competitive weekly pay: $2,422
