Job Description

Overview:
Tehachapi ADHC is seeking an Occupational Therapist to join our team and support our participants with their care and well being. A. Qualifications 1. The occupational therapist (OT) is a graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association and is licensed by the Occupational Therapy Board of California. The center has been provided with a copy of both the degree and the current registration. 2. The OT will provide the center with a health examination, signed by a physician, and a TB clearance, which is not more than 12 months prior to employment or within 7 days of employment and shall provide the center a current certificate of CPR and First Aid training. B. Duties The duties of the OT include: 1. Designing the occupational therapy program for the center and defining the responsibilities of the occupational therapist and any auxiliary personnel, i.e. occupational therapy assistants and/or aides. 2. Attending and participating in all the multidisciplinary team (MDT) meetings and subsequent MDT meetings if OT services are provided by the OT or recommended by the RN or physician. 3. Develop the plan of care in collaboration with the participant's identified goals and/or concerns. 4. Completing, for each participant, at minimum, an initial assessment, quarterly and six-month reassessments which includes evaluating and reviewing the progress made towards the participant's identified goal(s) collaboratively with participant and updating the plan of care as needed or desired by the participant and the collaborative process. a. Additional reassessments/updating of the IPC may also be completed in addition to the quarterly and six-month reassessments for the following reasons: when participant or his or her representative request, following inpatient hospital stay, after being away from the center following 30 days absence or a significant change in the participant's status, such as new diagnosis, new home environment, caregiving support, increased impairment/concerns inparticipant's mental/physical/emotional symptomology/status. 5. Completing reassessments/updating IPC when participant or his or her representative request, following inpatient hospital stay, after being away from the center following 30 days absence or a significant change in the participant's condition or functional status. 6. Determining if a maintenance program is indicated for each participant and, if so, developing the specific elements of the maintenance program, i.e. the problem(s), treatment(s) and objective(s) and preparing the participant's individual plan of care for the maintenance program. 7. Providing or supervising the provision of all occupational therapy specified in the participant plans of care. This may include, but is not limited to: a. Decreasing or eliminating disability during a participant's initial phase of recovery following injury or illness. b. Increasing or maintaining a participant's capability for independence. c. Enhancing a participant's physical, emotional and social well-being. d. Developing function to a maximum level. e. Guiding participants in the use of therapeutic, creative and self-care activities for improving function, such as leatherwork, weaving, needlework, ceramics, woodworking, painting and graphic arts. f. Therapeutic exercises, sensory stimulation and coordinating exercise concentrating on the range of motion of the upper extremities. g. Evaluation of the need for splints or slings to increase or maintain functional use of the upper extremities and preparation of these devices, should they be deemed necessary. h. Assistance in adapting equipment for re-education in the activities of daily living. 8. Providing training to family members and other staff members for those parts of the treatment plan that can be carried out by them. 9. Supervising any maintenance program(s) designated to be under the OT's supervision. 10. When indicated by the needs of a participant, providing information to the activity coordinator regarding appropriate therapeutic recreation and creative activities. 11. Maintaining the occupational therapy component of the participant health records, which includes: a. Documentation of all occupational therapy treatments provided, including the participant's reaction to treatment, on the day the service(s) is given. b. Countersigning all daily notes and/or flowchart entries documenting provision of a maintenance program, if the OT is designated as the supervisor. c. Quarterly progress notes for all participants receiving occupational therapy services, and for all participants receiving maintenance program services, if the OT is designated as the supervisor. d. Documentation of all initial assessments and, at least every six-months, any reassessments for occupational therapy. e. Documentation of all recommendations for maintenance programs and the specific elements of these programs. f. Documentation of all individual plans of care developed for occupational therapy and for maintenance programs, if the OT is designated as the supervisor. g. Documentation of any marked changes in a participant's condition or unusual incidents reported to or observed by the OT. 11. Training and supervising the staff, including the RN who is supervising the maintenance program and the staff that are providing the maintenance program 12. The OT works the hours necessary to meet the needs of the participants based upon their plans of care and to meet the requirements specified in Title 22, California Code of Regulations, Sections 54423 and 78419. 13. The OT reports to the Program Director. 14. Will be knowledgeable of
ADHC/CBAS
laws, regulations, CBAS certification standards, standards of participation and policies and procedures. This will be ensured through ongoing staff training and education.
Pay:
$55.00 - $75.00 per hour Expected hours: 6.0 - 20.0 per week
