This is a unique opportunity for a therapist who enjoys building long-term relationships with patients and wants to play a pivotal role in growing a caseload.
Key Responsibilities Comprehensive Evaluations:
Conduct thorough initial assessments of residents to determine functional limitations and rehabilitation potential.
Individualized Care:
Develop and implement evidence-based treatment plans tailored to the geriatric population, focusing on fall prevention, mobility, and pain management.
Collaborative Care:
Work closely with the facility's families, and physicians to ensure a holistic approach to resident wellness.
Documentation:
Maintain accurate and timely electronic medical records (EMR) in compliance with state and federal regulations.
Program Growth:
Act as an ambassador for the therapy department, educating residents and staff on the benefits of outpatient PT to help grow the caseload toward a full-time schedule.
Required Qualifications Education:
Graduate of an accredited Occupational Therapy program.
Licensure:
Current and unrestricted Physical Therapy license in the state of California.
Experience:
A minimum of 1 year of experience specifically working with the geriatric population (Outpatient, SNF, or ALF experience preferred).
Skills:
Strong clinical reasoning, excellent interpersonal communication, and a "growth mindset" to help build the program.
