In this vital role, you will assess, plan, and implement personalized therapy programs to improve client's functional abilities across a variety of settings. Your expertise will help transform lives through innovative therapeutic interventions and compassionate care. Duties Conduct comprehensive assessments of patients' physical, cognitive, and emotional needs to develop tailored treatment plans. Collaborate with multidisciplinary teams to coordinate care plans and ensure seamless patient transitions across care settings. Provide therapy services in diverse environments including inpatient units, outpatient clinics, skilled nursing facilities, schools, and home health visits. Engage in discharge planning by preparing detailed reports and recommendations for ongoing therapy or community support. Work with individuals with disabilities and autism spectrum disorders to enhance motor skills, sensory processing, and social participation. Maintain accurate documentation of patient progress, treatment interventions, and case management activities in compliance with healthcare regulations. Stay current with medical terminology, physiology knowledge, and anatomy to inform effective therapeutic strategies. Requirements Valid licensure as an Occupational Therapist in CT Demonstrated ability to work with diverse pediatric populations including Strong knowledge of medical terminology, physiology, and anatomy to support accurate assessments and interventions. Ability to manage case loads efficiently while delivering compassionate patient care. Excellent communication skills for collaborating with healthcare teams and educating patients and families about therapy goals.
