Responsibilities:
Conduct comprehensive evaluations of residents' physical abilities, limitations, and rehabilitation needs, focusing on improving overall function, mobility, and strength. Develop and implement individualized treatment plans based on each resident's unique needs, goals, and abilities, in alignment with their overall care plan. Provide outpatient therapy services , including therapeutic exercises, manual therapy, and modalities to improve strength, balance, and flexibility. Monitor and track patient progress , making adjustments to treatment plans as needed to ensure optimal outcomes. Collaborate with interdisciplinary healthcare teams (doctors, nurses, caregivers) to ensure cohesive and effective care for all residents. Educate residents and their families about therapeutic exercises, home care strategies, and preventative techniques to enhance long-term wellness. Maintain accurate, up-to-date documentation of evaluations, treatments, progress, and patient outcomes in line with facility guidelines and regulatory standards. Foster a positive and supportive environment where residents feel comfortable and motivated to engage in their rehabilitation.
Qualifications:
Degree in Occupational Therapy from an accredited institution. Valid state license to practice occupational therapy. Previous experience in outpatient therapy or assisted living settings preferred Strong knowledge of geriatrics, rehabilitation techniques , and treatment modalities for older adults. Excellent communication skills for building relationships with patients, families, and the healthcare team. A compassionate and patient-centered approach , with a passion for working with older adults to enhance their quality of life. Ability to work independently and as part of a team in a collaborative, fast-paced environment.
