Job Description
Tender Touch Rehab Services seeks a licensed therapist to provide high-quality rehabilitation services to residents at Aviata Green Cove Springs. In this role, you'll evaluate geriatric and sub-acute patients, create individualized treatment plans, and deliver physical, occupational, or speech therapy focused on maximizing function, safety, and quality of life. You'll collaborate with an interdisciplinary team, document outcomes, and participate in ongoing education. Responsibilities Provide physical, occupational, or speech therapy services to geriatric and sub-acute residents in a skilled nursing setting Complete comprehensive resident assessments and develop individualized treatment plans Deliver evidence-based interventions to improve mobility, function, and quality of life Collaborate closely with an interdisciplinary team and facility staff on care planning Document treatments, progress notes, and outcomes in compliance with regulations Educate residents, families, and caregivers on therapy goals and home exercise programs Participate in case conferences, quality improvement, and in-service training Maintain professional, ethical, and compassionate resident-centered care Mentor or support therapy teammates as needed Required Skills Geriatric rehabilitation Skilled nursing facility rehabilitation Patient evaluation and assessment Therapy treatment planning Physical therapy interventions Occupational therapy interventions Speech-language therapy techniques Clinical documentation and EMR use Interdisciplinary team collaboration Patient and family education
