VitalCaring is seeking an Occupational Therapist to provide home-based therapy for adult and geriatric patients. In this role, you will assess functional abilities, develop individualized treatment plans, and deliver evidence-based interventions to improve safety, mobility, and independence at home. You will educate patients and caregivers, coordinate with nurses, physicians, and social workers, and document care in accordance with regulatory and agency standards. Responsibilities Provide in-home occupational therapy evaluations and ongoing treatment Develop and update individualized care plans based on patient goals and functional status Implement therapeutic exercises, ADL training, and home safety interventions Educate patients and caregivers on techniques, equipment use, and home modifications Collaborate with nurses, physicians, and interdisciplinary team members on care coordination Maintain accurate, timely clinical documentation per regulatory and agency standards Monitor patient progress, adjust treatment plans, and communicate changes to providers Participate in case conferences, quality initiatives, and clinical training Ensure compliance with state, federal, and payer requirements for home health and hospice Promote patient safety, dignity, and independence in the home setting Required Skills Occupational therapy evaluation and treatment Home health clinical practice Activities of daily living (ADL) training Home safety assessment and modification Geriatric and adult rehabilitation Caregiver education and training Interdisciplinary care coordination Electronic health record (EHR) documentation Knowledge of Medicare and home health regulations Use of adaptive equipment and assistive devices
