What You'll Do:
Conduct comprehensive assessments in the patients home, to evaluate patients' functional abilities and daily living challenges. Develop and implement evidence-based treatment plans tailored to each patient's unique needs and goals. Utilize therapeutic interventions to promote independence in self-care, mobility, and daily tasks. Educate patients and caregivers on adaptive strategies, assistive devices, and safety techniques for enhanced functionality. Collaborate closely with other healthcare professionals to ensure holistic, patient-centered care. Maintain accurate, compliant documentation of patient care and progress.
What We're Looking For:
Licensed Occupational Therapist (OT) in good standing with the National Board for Certification in Occupational Therapy (NBCOT). A minimum of 1 year of clinical experience, ideally in home health or a related field. Strong clinical reasoning skills and a patient-centered approach. Ability to manage time effectively and work independently within a flexible schedule. Reliable transportation for patient visits. Why Axiom Rehab?
