Job Description

Occupational Therapist (OT) - Out Patient in the Home Upgrade Physical Therapy & Fitness is seeking a compassionate, motivated, and patient-centered Occupational Therapist (OT) to join our growing mobile outpatient practice serving older adults in their homes. You will be treating patient in the Ocoee, Winter Garden and Surrounding Areas. This is an excellent opportunity for an OT who enjoys building meaningful relationships with patients while providing high-quality, one-on-one care without the productivity pressures of traditional home health. About the Role As an Occupational Therapist, you will evaluate and treat patients in their homes, helping them improve independence, safety, and quality of life. You will collaborate closely with our physical therapists and other providers to deliver exceptional interdisciplinary care. Responsibilities Perform comprehensive occupational therapy evaluations and reassessments. Develop individualized treatment plans based on each patient's goals. Provide skilled interventions focused on ADLs, IADLs, upper extremity function, cognition, home safety, fall prevention, and adaptive equipment. Educate patients and caregivers to promote independence and long-term success. Complete accurate and timely documentation using our Jane EMR. Communicate effectively with physicians and other healthcare providers regarding patient progress. Qualifications Active Florida Occupational Therapist license. Medicare documentation experience preferred. Excellent communication and interpersonal skills. Valid driver's license and reliable transportation. Current CPR certification.
