Job Summary We are seeking a dedicated and energetic PT, OT, and ST Therapist to join our dynamic healthcare team. Duties Conduct thorough assessments of patients' physical, neurological, and functional status to develop personalized care plans. Collaborate closely with physicians, nurses, and other healthcare providers to coordinate discharge planning and ensure seamless patient care transitions. Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR) systems. Monitor patient progress regularly, adjusting treatment plans to optimize outcomes for conditions such as orthopedics, neurology, pediatrics, and geriatrics. Educate patients and their families on treatment goals, home exercises, safety measures, and community resources to support ongoing recovery. Ensure compliance with clinical documentation standards and participate in quality improvement initiatives related to outpatient rehabilitation centers, home health services, or inpatient hospital settings. Experience Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospital units, or home health environments. Demonstrated expertise in discharge planning and working with geriatric patients in various care settings. Hands-on experience with outpatient rehabilitation center protocols and inpatient care procedures. Knowledge of physiology, anatomy, medical terminology, and patient rehabilitation techniques across diverse specialties including orthopedics and neurology. Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for accurate documentation of patient care activities. Strong communication skills for effective client interaction and collaboration with healthcare teams.
