Job Description
Job Summary Madison Health and Rehab is seeking dedicated and compassionate Occupational Therapists to join our team. As part of our Skilled Nursing Program, our occupational therapists are key to increasig the rehab potential of our short term residents and also the continued ability of our long term residents. The ideal candidates will have experience in various settings, including skilled nursing facilities, geriatrics, outpatient care, and acute care environments. As an Occupational Therapist, you will play a crucial role in helping patients regain their independence and improve their quality of life through therapeutic interventions and personalized care plans. Responsibilities Conduct comprehensive assessments of patients' physical, emotional, and social needs to develop individualized treatment plans. Implement therapeutic interventions aimed at improving patients' ability to perform daily activities. Collaborate with multidisciplinary teams to ensure coordinated patient care and effective discharge planning. Educate patients and their families about treatment processes, goals, and expected outcomes. Maintain accurate documentation of patient progress and treatment outcomes in compliance with medical terminology standards. Stay updated on best practices in occupational therapy and participate in ongoing professional development. Requirements A degree in Occupational Therapy from an accredited program. Valid state licensure or certification as required by local regulations. Experience in skilled nursing facilities, geriatrics, outpatient settings, or acute care is preferred. Excellent communication skills to effectively interact with patients, families, and healthcare professionals. Ability to work collaboratively within a team-oriented environment while demonstrating empathy and patience.
