Position Summary:

The OT provides initial and ongoing assessments to determine level of functioning and develop a plan of care to patients in their place of residence. This is performed in accordance with physician orders and IDT plan of care under the direction and supervision of the Agency Supervisory.

Responsibilities:

• Developing and revising the plan of care, including OASIS assessments, at the appropriate time and in consultation with other care-team members.
• Communicate effectively with all other health team personnel including but not limited to; administration, certified occupational therapy assistant (COTAs), home health aides (HHAs), case management, nursing, physical therapy, physical therapist assistants, speech therapy, medical social workers, and family.
• Confirm the scheduling of visits with team members to coordinate necessary visits with other personnel.

Qualifications:

• Graduated after successful completion of an OT education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organization of ACOTE; and
• Is eligible to take, or has successfully completed the entry-level certification examination for OT developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).
• BLS•Reliable transportation with current insurance coverage and driver's license•Experience with OASIS and Wellsky is a plus•Experience working in home health or similar setting preferred
