What You'll Do As a Home Health Occupational Therapist, you'll: Complete in-home OT evaluations and visits Develop individualized treatment plans Help patients improve independence with daily activities Recommend adaptive strategies, equipment and home modifications as appropriate Educate patients and caregivers Monitor progress and update treatment plans Complete required clinical documentation Collaborate with the patient's care team as needed

What We're Looking For Required:

Current Illinois Occupational Therapist license Reliable transportation Ability to travel to patient homes within your selected coverage area Strong communication and documentation skills Home health experience is preferred but not required. OTs with rehabilitation, skilled nursing, outpatient or acute care experience are encouraged to apply.
