2987669958-2 Apply for this job now Job Description LifePoint Health is seeking an Occupational Therapist to deliver high-quality, patient-centered rehabilitation services in a community-based setting. In this role, you will evaluate patients, develop and implement individualized treatment plans, and help patients regain functional independence in daily activities. You will collaborate closely with an interdisciplinary care team, educate patients and families, and document progress in accordance with clinical and regulatory standards. Responsibilities Evaluate patients' functional abilities and limitations through standardized assessments Develop and implement individualized occupational therapy treatment plans Provide therapeutic interventions to improve ADLs, mobility, and functional independence Collaborate with physicians, nurses, and other rehab professionals on care plans Educate patients and families on home exercise programs and adaptive strategies Document evaluations, treatments, and progress in the medical record accurately and timely Ensure compliance with regulatory, safety, and clinical quality standards Participate in performance improvement and evidence-based practice initiatives Required Skills Occupational therapy evaluation and treatment planning Knowledge of ADL and functional mobility training Therapeutic exercise and neuromuscular re-education Use of adaptive equipment and assistive technology Electronic health record (EHR) documentation Interdisciplinary care coordination Patient and family education Regulatory and reimbursement knowledge (CMS, documentation standards) Evidence-based rehabilitation techniques Time management and caseload prioritization
