Complete appropriate goal re-assessment and discharge planning. Provide occupational therapy, according to standards of care defined by the state practice act and other professional and governing agencies. Perform comprehensive occupational therapy evaluations, treatment and related follow-up services, including discharge planning. Document and submit patient reports and charges within the established timeframe. Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care. Develop a treatment plan with measurable goals based on patient assessment. Provide occupational therapy, including selection and use of assistive equipment. Train and educate patients and family members or other support, and document understanding of treatment plan and goals.QUALIFICATIONS
Required Bachelor's Degree or Master's Degree in Occupational Therapist Required within 90 days of hire Occupational Therapist (OT)- State Licensing Board Indiana or Illinois Required Cardiopulmonary Resuscitation (CPR)
- American Heart Association Required Cardiopulmonary Resuscitation (CPR)
- American Heart Association Preferred experience in Occupational Therapy Preferred Doctorate Occupational Therapist Preferred 1 year Occupational Therapy
