Job Description

You'll work alongside a rehab team that collaborates across disciplines and a Director of Rehabilitation who supports the work. What we offer

  • Assess referred residents who need rehabilitation services
  • Evaluate medical condition, functional capability, limitations, and rehab potential
  • Build and run treatment programs under the direction of the physician
  • Set realistic, achievable goals matched to each resident's capacity and tolerance
  • Complete evaluations, weekly progress notes, recertification, and discharge documentation
  • Educate residents and families on therapy methods they can continue
  • Collaborate across disciplines to deliver resident-centered care
  • Follow confidentiality, HIPAA, and Federal Resident Rights requirements What you need
  • Graduate of an accredited Occupational Therapy college or university
  • Valid professional license and registration for the state of practice
  • Long-term care or geriatric experience preferred Ready to make a difference?
