Per Visit Rates:
Oasis Recertification/Resumption of Care $90/visit OT Evaluation $85/visit Routine follow up visit $75/visit Elite Home Rehab and Healthcare is an accredited ACHC skilled home health agency servicing Central Indiana. We are a clinician owned agency, dedicated to providing quality care to our patients as well as helping our clinicians grow professionally. We are looking for a part time Occupational Therapist for Greenwood, IN and surrounding areas.Duties of Position:
Provide skilled occupational therapy services on an intermittent basis to patients in their home in accordance with the physician's orders.Position Responsibilities:
Evaluate the patient's level of functioning and recommends mechanisms to enhance the patient's ADL ability. Provided initial and ongoing comprehensive assessments of the patient's needs in including Outcome and Assessment Information Set (OASIS) assessments. Establish a Occupational Therapy treatment plan in consultation with the physician and revises it with the approval of the physician as necessary. Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Occupational Therapy. Teach, supervise and counsel the family and patient in the total Occupational Therapy program and other related problems of the patient at home. Record on a timely basis all evaluation data, treatments, and patient's reaction to treatment and/or any changes in the patient's condition. Communicate with the physician at least every 30 days or whenever changes occur regarding the patient's status and obtains additional orders, if necessary. Participate in agency activities, case conferences, and meetings, when appropriate. Participate with staff, patients, and physicians in discharge planning activities and completes a Occupational Therapy Discharge Summary.Qualifications:
Must have the ability to drive and access patients' homes. Licensed in the State of Indiana to practice Occupational Therapy. Preferred to have at least one year of institutional experience. Must complete a criminal background check. Must have a current CPR certification.Job Types:
