Community Rehabilitation Hospital South seeks an Occupational Therapist to provide evidence-based rehabilitative care to adult patients recovering from serious illness or injury. In this role, you will assess functional abilities, develop individualized treatment plans, and implement interventions that promote independence in daily living. You will collaborate closely with an interprofessional team, educate patients and families, and document progress in the EMR. Responsibilities Evaluate patients' functional abilities, limitations, and rehabilitation needs. Develop and implement individualized occupational therapy treatment plans. Provide interventions to improve ADLs, mobility, and functional independence. Select and train patients in the use of adaptive equipment and assistive devices. Collaborate with physicians, nurses, and other therapists on coordinated care plans. Document assessments, goals, and progress accurately in the EMR system. Educate patients and families on home programs and strategies to maintain gains. Participate in interdisciplinary team meetings and discharge planning. Adhere to regulatory, safety, and professional practice standards. Engage in ongoing professional development and quality improvement initiatives.
