Duties Conduct comprehensive evaluations of patients' physical and functional status, including assessments related to ADLs (Activities of Daily Living) and mobility. Develop and implement customized care plans that address individual needs, goals, and medical conditions. Assist patients with ADLs such as dressing, grooming, feeding, and other essential daily tasks to foster independence. Coordinate discharge planning with healthcare teams to ensure seamless transitions from acute or inpatient settings to home-based care. Collaborate with families, caregivers, and interdisciplinary teams to educate on safety, adaptive techniques, and ongoing therapy needs. Manage case documentation meticulously, including progress notes, treatment plans, and outcome reports. Participate in outpatient clinics or early intervention programs when required, providing specialized therapy for pediatrics or individuals with disabilities. Requirements Valid occupational therapy license Previous Home Health experience CPR certified

$60.00 - $70.00 per hour
