Tender Touch Rehab Services is seeking a Licensed Occupational Therapist to provide compassionate, high-quality care to geriatric and sub-acute residents in skilled nursing and long-term care facilities. In this field-based role, you will perform comprehensive evaluations, design individualized treatment plans, and deliver evidence-based interventions that maximize functional independence, safety, and quality of life. You will collaborate closely with an interdisciplinary team, participate in care plan meetings, and maintain accurate, timely documentation. Responsibilities Provide skilled occupational therapy evaluations and treatments for geriatric and sub-acute residents in skilled nursing and long-term care facilities Develop, implement, and adjust individualized care plans focused on functional independence, safety, and quality of life Collaborate closely with an interdisciplinary team including PT, SLP, nursing, and physicians to coordinate resident care Educate residents, families, and facility staff on therapeutic activities, adaptive equipment, and safe performance of daily living tasks Document evaluations, daily treatments, progress notes, and discharge summaries in accordance with professional and regulatory standards Participate in case conferences, care plan meetings, and quality improvement initiatives within partner facilities Adhere to ethical, regulatory, and payer guidelines while maintaining high standards of clinical excellence Contribute to a collaborative, patient-focused culture through open communication and professional respect Engage in ongoing professional development and utilize clinical support resources provided by Tender Touch Rehab Services Required Skills Occupational therapy evaluation and treatment planning Geriatric and sub-acute rehabilitation ADL and IADL retraining Functional mobility and balance training Therapeutic exercise and interventions Splinting and adaptive equipment training Clinical documentation and EMR proficiency Interdisciplinary team collaboration Patient and family education Knowledge of SNF/LTC regulations and reimbursement
