Skilled nursing facility (SNF) Responsibilities Perform comprehensive occupational therapy evaluations for adult residents in a skilled nursing setting. Develop and implement individualized treatment plans to improve activities of daily living, mobility, cognition, and hand function. Deliver direct therapy and supervise related tasks to support residents' rehabilitation goals. Regularly reassess progress, adjust goals as needed, and document outcomes in the electronic health record. Collaborate with an interdisciplinary team (therapy, nursing, social work, and care management) to support discharge planning and ensure continuity of care. Educate residents and families on home programs, safety strategies, and adaptive equipment as appropriate. Maintain clinical standards, adhere to regulatory requirements, and monitor patient safety and infection control. Requirements Active or eligible Occupational Therapist license to practice in the United States; license verification required. Ability to work in a skilled nursing facility environment and collaborate with an interdisciplinary team. Strong assessment, treatment planning, and documentation skills. Excellent communication with residents, families, and colleagues; ability to document clearly and timely. Availability to commit to a 13-week travel assignment starting November 23, 2026.
