Job Description

Renewal Rehab is seeking an Occupational Therapist to provide high-quality, patient-centered care to seniors in skilled nursing and post-acute care settings. In this role, you will assess functional abilities, create evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life. You'll collaborate closely with an interdisciplinary team, maintain accurate documentation, and educate patients, families, and staff. Responsibilities Evaluate senior patients in skilled nursing and post-acute settings, using standardized OT assessments Develop and implement individualized, evidence-based treatment plans focused on function and safety Provide therapeutic interventions to improve ADLs, mobility, strength, balance, and cognition Document evaluations, daily notes, and progress reports in EMR accurately and timely Collaborate with PT, SLP, nursing, and physicians on care plans and discharge recommendations Educate patients, families, and facility staff on home programs, safety, and adaptive strategies Participate in care conferences, quality initiatives, and outcome tracking Support a patient-centered culture aligned with integrity, respect, and clinical excellence Required Skills Adult and geriatric assessment ADL and IADL retraining Functional mobility and transfer training Splinting and positioning Cognitive and perceptual rehab Therapeutic exercise and neuromuscular re-education Care planning and documentation (EMR) Interdisciplinary team collaboration Patient and family education Discharge planning and safety evaluation
