What You'll Do:
As an Occupational Therapist at Thome PACE, you'll play a vital role in empowering older adults to live as independently, safely, and confidently as possible. You'll provide hands-on occupational therapy services in our welcoming Day Center and in participants' homes, adapting interventions to each individual's unique strengths, needs, and environment. Key Responsibilities Conduct initial assessments and reassessments, upon referral from the team, of participants' conditions and functional status. Communicate any changes in the participant's condition to team members promptly. Develop individualized treatment programs as needed, setting specific, measurable goals for improvement. Provide therapeutic interventions and treatments based on the established treatment program. Record and sign treatments in participants' medical records, including signing and dating "Progress Notes." Coordinate 24-hour care delivery to ensure comprehensive support for participants. Assess the need for adaptive and durable medical equipment to aid participants in achieving optimal independence. Work with the Home Care Coordinator to facilitate the purchase of necessary durable medical equipment. Review assessments and discharge participants when appropriate. Provide training to family members, friends, and Thome PACE staff on parts of the treatment plan that can be carried out by them. Supervise program aides and other staff members who are implementing the OT treatment program to ensure effective service delivery. Actively participate as a member of the Interdisciplinary Team, attending staff meetings, and contributing to participant care planning. Participate in case conferences when necessary to discuss and review participant care. Skills, Knowledge and Expertise Graduate of an Occupational Therapy curriculum accredited by the Accreditation Council for Occupational Therapy Education (ACOTE). Licensed Occupational Therapist in the State of Michigan. Must be CPR certified or willing to obtain. Must possess a valid and current driver's license and have reliable transportation. Minimum of two (2) years of experience as an Occupational Therapist. At least one (1) year of experience working with frail and elderly populations is required. Strong assessment and intervention skills in occupational therapy, particularly for elderly or frail individuals. Ability to develop and implement individualized therapy plans focused on improving daily living skills and independence.
