[$60-$120 Per Visit] Position Summary We are seeking an Occupational Therapist to help patients improve their independence, safety, and participation in daily activities at home. The OT evaluates each patient's functional abilities, develops an individualized treatment plan, and works with patients, caregivers, and the care team to achieve meaningful goals. Responsibilities Evaluate patients' ability to perform daily activities, including dressing, bathing, grooming, toileting, and meal preparation. Develop and implement individualized plans of care based on patients' needs, goals, and home environments. Provide skilled interventions to improve functional mobility, strength, coordination, cognition, and daily living skills. Assess home safety and recommend adaptive equipment, assistive devices, and environmental modifications. Teach patients and caregivers safe techniques, energy conservation strategies, and home exercise programs. Collaborate with physicians, nurses, therapists, and other care team members to coordinate care. Complete accurate, timely evaluations, visit notes, progress reports, and discharge documentation in the electronic medical record. Monitor patient progress and adjust interventions as appropriate. Supervise occupational therapy assistants when assigned, consistent with applicable requirements. Follow agency policies, infection prevention practices, and patient privacy standards.
Qualifications Required:
Graduation from an accredited occupational therapy program. Current, unrestricted Occupational Therapist license in the state of practice. Current CPR/BLS certification or ability to obtain before starting. Strong clinical judgment, communication, and organizational skills. Ability to work independently and manage a schedule of home visits. Reliable transportation and ability to travel throughout the assigned service area. Ability to perform essential job duties, with or without reasonable accommodation.
Preferred:
Previous home health, geriatric, or rehabilitation experience. Experience with electronic documentation and interdisciplinary care. Familiarity with adaptive equipment, caregiver education, and home safety assessments.
