They're important, but they rarely tell you everything you need to know. Then you walk through the patient's front door. You see the stairs. You meet the caregiver. You notice the patient isn't moving quite like the referral suggested. You find out they're sleeping in a recliner because they can't get into bed. The medication setup isn't as clear as expected. The spouse who's listed as "available to assist" may have limitations of their own. Suddenly, the clinical picture looks very different. You're the First Set of Clinical Eyes in the Home Home Health Care, Inc. is seeking an experienced Occupational Therapist (OTR/L) to complete Start of Care admissions throughout the Twin Cities Metro and surrounding communities. This isn't a traditional treatment-focused OT position. Your value comes from your ability to assess, investigate, prioritize, document, and make strong clinical decisions from the beginning. You'll take the information provided at referral and determine what the patient actually needs now that they're home. A Lot of Clinical Decisions. A Start of Care visit requires you to see more than what's immediately obvious. Can the patient safely complete daily activities? What does their functional mobility actually look like? Are cognitive changes affecting safety? Does the caregiver understand what will be required? Is the home environment creating risks? What equipment is missing? Which disciplines need to be involved?
And perhaps most importantly:
what needs attention first? You're gathering those pieces and creating a clinical picture the rest of the home health team can act on. What You'll Be Doing Complete comprehensive Start of Care Occupational Therapy evaluations in patient homes Assess ADLs, cognition, functional mobility, upper-extremity function, caregiver support, and home safety Complete OASIS and admission documentation accurately and within required timeframes Identify immediate clinical, functional, and environmental priorities Develop individualized plans of care based on findings, physician orders, and patient goals Identify fall risks and barriers to safe independence Recommend adaptive equipment and appropriate home modifications Educate patients and caregivers on safety and adaptive strategies Coordinate with nursing, PT, SLP, social work, physicians, and other clinicians Complete timely electronic documentation and maintain regulatory compliance Travel throughout the Twin Cities Metro and surrounding service area based on referral needs This Isn't a Role Where Someone Else Has Already Figured Everything Out There may not be a previous therapy note telling you how the patient performed last week. There may not be an established routine. You're establishing the baseline. That's why we're looking for an experienced clinician who can walk into an unfamiliar home, establish rapport quickly, ask the right questions, recognize what doesn't add up, and confidently determine what needs to happen next. What You Bring Current Minnesota Occupational Therapist (OTR/L) license Current CPR certification Valid driver's license and reliable transportation Ability to travel throughout an approximately 75-mile service radius Strong assessment and clinical reasoning skills Excellent documentation and organization Ability to communicate effectively with patients, caregivers, physicians, and interdisciplinary clinicians Confidence working independently in unfamiliar patient environments Especially Valuable Experience Previous home health experience OASIS experience Start of Care or admission experience Knowledge of Medicare home health requirements Experience with medically complex, orthopedic, neurological, or geriatric populations Optional Weekends Give You Another Earning Lever Your primary role is during the week, but if you want to increase your earnings, weekend Start of Care visits may be available at a higher rate. Weekday admissions pay $100 per Start of Care visit , while optional weekend admissions pay $120 per visit . Pick them up when the opportunity and your availability align, without making weekend coverage the centerpiece of your regular schedule.
