3005982514-2 Apply for this job now Job Description Essentia Health seeks a compassionate Occupational Therapist to provide patient-centered rehabilitation services within our integrated Healthcare & Medical Services team. In this role, you will assess functional needs, develop evidence-based treatment plans, and deliver individualized interventions to help patients maximize independence in daily activities. You'll collaborate with a multidisciplinary team, educate patients and families, and contribute to quality improvement initiatives. Responsibilities Provide comprehensive occupational therapy evaluations and individualized treatment plans for patients across the continuum of care Deliver evidence-based interventions to improve functional independence in self-care, work, and leisure activities Collaborate closely with interprofessional teams, patients, and families to support holistic, patient-centered care Document assessments, goals, and progress accurately in the electronic health record and maintain regulatory compliance Educate patients, caregivers, and staff on adaptive strategies, equipment use, and home exercise programs Participate in quality improvement, clinical education, and continuous learning initiatives within the department Contribute to a culture of compassion, collaboration, and community engagement aligned with Essentia Health values Required Skills Occupational therapy evaluation and treatment planning Activities of daily living (ADL) and instrumental ADL training Neuromuscular and orthopedic rehabilitation techniques Functional cognition and perceptual assessment Assistive technology and adaptive equipment selection and training Splinting and upper extremity rehabilitation Patient and family education Interdisciplinary team collaboration and care coordination Electronic health record (EHR) documentation Outcome measurement and evidence-based practice
