Job Description
- Conduct patient assessments to determine needs and develop individualized treatment plans
- Help patients improve their ability to perform daily tasks and activities
- Educate patients and their families on techniques for continued improvement at home
- Collaborate with other healthcare professionals to provide comprehensive care
- Maintain accurate patient records and progress reports Experience
- Previous experience in geriatrics, acute care, clinic, ICU, outpatient settings is preferred
- Knowledge of physiology and medical terminology is beneficial
