Job Description

Job Summary:
Park River Healthcare and Rehabilitation Center is seeking a dedicated and compassionate Occupational Therapist (OT) to join our interdisciplinary team in a long-term care and rehabilitation setting. The Occupational Therapist will evaluate residents and develop individualized treatment plans that help improve independence, functional abilities, and overall quality of life. This role focuses on restoring and maintaining residents' ability to perform activities of daily living following illness, injury, or disability. This position will be primarily based at Park River Healthcare and Rehabilitation Center and will float to New Brighton Care Center as needed to complete therapy evaluations.
Primary Responsibilities:
Evaluate residents' physical, cognitive, and functional abilities to determine occupational therapy needs. Develop and implement individualized occupational therapy treatment plans. Provide therapeutic interventions to improve residents' ability to perform activities of daily living (ADLs) such as dressing, bathing, eating, and mobility. Complete therapy evaluations at New Brighton Care Center as scheduled or as needed. Collaborate with physicians, physical therapists, speech therapists, nursing staff, and other healthcare professionals to coordinate care. Educate residents, families, and caregivers on therapy techniques, adaptive equipment, and safety strategies. Maintain accurate and timely documentation in accordance with facility policies, state regulations, and Medicare/Medicaid guidelines. Monitor resident progress and adjust treatment plans as necessary.
Qualifications:
Current Occupational Therapist license in the applicable state. Graduate of an accredited Occupational Therapy program. Experience in long-term care, skilled nursing, or rehabilitation setting preferred. Strong communication and interpersonal skills. Ability to work effectively with an interdisciplinary team. Ability to travel between Park River Healthcare and Rehabilitation Center and New Brighton Care Center as needed. Commitment to providing compassionate, resident-centered care. Ensure compliance with all regulatory, safety, and infection control standards.
Pay:
$50.00 - $56.00 per hour
