At Sovereign, we are looking for an extraordinary Occupational Therapist to provide dedicated care to the residents we are serving in Excelsior Springs, MO . Responsibilities Identify and meet patient goals and needs based on holistic assessment and functional reassessments during patient care Conduct, evaluate and document a thorough functional assessment/evaluation of the patient/resident, utilizing standardized procedures, with use of point of service documentation when appropriate. Assessment of home/vocational and equipment needs when appropriate Develop and implement an individualized treatment plan based on resident needs and containing achievable functional goals Document the course of treatment in a clear concise discharge summary utilizing a prescribed format Work with nursing staff, resident and families to maximize resident's use of functional abilities Assist and support on weekends, as needed, in order to meet patient needs and Medicare standards for seven day a week therapy provision Provide quality, personalized and evidence-based care and proven interventions Instruct residents and family/other caregiver about exercise programs and therapeutic procedures to be continued by the resident at home following treatment Instruct Assistants in treatment procedures to be utilized and provide supervision while such staff members are working with patient/residents Attend and participate in, or provide information for rehabilitation team conferences regarding resident progress, problems or needs Attend and participate in other department/facility meetings, along with marketing and sales team as required Orient residents to program services and treatment procedures appropriate to their needs. Inform residents of any potential risk during any procedures with documentation recorded in the EMR System Maintain the expected productivity level that has been discussed and is expected for your position at your particular facility/facilities Skills Proven work experience as an Occupational Therapist Act in compliance with national and state regulatory and professional standards and guidelines Current knowledge of treatment practices Demonstrate a knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third party reimbursement guidelines Exhibit Professional behavior at all times Excellent oral and written communication and interpersonal skills Valid, active license to practice physical therapy within the state of hire Bachelor/Masters/Doctorate of Science degree in

Highly competitive, subject to discussion during hiring process, and based on experience/qualifications.
