Long-term care nursing home Responsibilities Conduct comprehensive OT evaluations and develop treatment plans to address activities of daily living, mobility, safety, and functional performance in a LTC environment. Deliver individualized, goal-focused rehabilitation interventions; monitor progress and adjust goals in collaboration with the multidisciplinary team. Document assessments, treatment plans, progress notes, and discharges in the resident's medical record in compliance with regulatory standards. Collaborate with physicians, nursing staff, therapy teammates, and case managers to support discharge planning and continuity of care. Educate residents and families on rehabilitation strategies, adaptive equipment, home safety, and strategies to maintain gains after discharge.
