Licensed Occupational Therapist Job Summary:
Homeside Rehab is seeking a compassionate and skilled Occupational Therapist to join our home health team. In this role, you will deliver individualized, evidence-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.
Job Responsibilities:
Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals Provide evidence-based OT interventions to improve motor skills, functional independence, and developmental outcomes Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements Collaborate with the clinical team to ensure coordinated and effective treatment Communicate clearly with office staff, patients, and caregivers regarding progress and care plans
Job Qualifications:
Active Nebraska State Occupational Therapy License - required Basic computer proficiency for electronic documentation Strong clinical assessment and interpersonal communication skills Ability to work independently in a home health environment Valid Nebraska State driver's license and reliable transportation for home visits Pediatric experience is preferred Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings
