We are looking for a Part-Time Occupational Therapist to join our growing medically integrated health care team. As a medically integrated office, we provide Occupational Therapy services as an option to our patients who needs a more specific, defined, and narrower focus of care. Responsibilities  Completes assessment and evaluation of patient's impairments and deficits  Working in partnership with our current team  Creates a treatment plan and goals to address these impairments and deficits with consideration of evaluation and assessment, medical status, and prior level of function. The treatment plan and goals are monitored and adjusted based on the patient's needs and progress  Instruct, encourage and assist patients in performing a therapeutic activity, therapeutic exercise, and activities of daily living, specific to each patient's needs and goals. Adjusts selected therapeutic activity, therapeutic exercise, and activities of daily living based on the patient's response to activity  Evaluate effects of treatments by observing, noting, and evaluating patient's progress then recommending adjustments and modifications in consultation with other providers  Prepares written documentation such as initial documentation, notes, reassessments, and discharge summaries. Documentation includes monitoring the resident's status and progress towards goal achievements. Re-evaluations are completed in a timely manner  Goals and treatment plans are updated as per department policy. Consistently, accurately, and independently prioritizes and completes routine assignments within guidelines and schedules Qualifications  New Jersey board-certified and licensed in Occupational Therapy (NBCOT)  Certified PAMS - preferred  Applicants must be energetic, positive people person and have the ability to combine clinical skills with genuine concern and empathy for their patients  Great communication skills and work well with others  Organized and able to multitask Compensation Starting at $55 per hour About Bloomfield Total Health Center Welcome to Bloomfield Total health Center.
