The OT will provide rehabilitative services to skilled residents with the goal of restoring functional independence, while also supporting long-term care residents through maintenance therapy programs designed to preserve current abilities, prevent decline, and maximize quality of life. The Occupational Therapist collaborates closely with nursing, therapy, medical providers, social services, and facility leadership to ensure residents receive high-quality, patient-centered care that complies with federal, state, and facility regulations. Essential Duties and ResponsibilitiesSkilled Rehabilitation Services Evaluate residents referred for occupational therapy services. Develop individualized treatment plans based on physician orders, clinical findings, and resident goals. Provide skilled occupational therapy interventions to improve: Activities of Daily Living (ADLs) Functional mobility Transfers and positioning Upper extremity strength and coordination Fine motor skills Cognitive and perceptual abilities Safety awareness Adaptive equipment use Establish realistic discharge goals and recommendations. Participate in interdisciplinary care planning and discharge planning meetings. Modify treatment plans based on resident progress and changing medical conditions. Long-Term Care & Maintenance Therapy Evaluate LTC residents for maintenance therapy eligibility.

Develop maintenance programs intended to:

Prevent functional decline Maintain resident mobility and independence Reduce risk of falls and injury Preserve upper extremity function and range of motion Support participation in meaningful daily activities Collaborate with nursing staff and restorative nursing programs to ensure resident carryover and ongoing support. Monitor and update maintenance therapy plans as clinically indicated. Documentation & Compliance Complete all evaluations, progress notes, recertifications, discharge summaries, maintenance therapy documentation, and daily treatment notes in a timely and accurate manner.

Maintain compliance with:

Medicare regulations Medicaid regulations State practice acts CMS requirements Facility policies and procedures Ensure documentation supports medical necessity and skilled need. Participate in quality assurance and performance improvement initiatives. Technology & Systems Demonstrate proficiency in: PointClickCare (PCC) for resident documentation and clinical review. NetHealth (formerly Rehab Optima) for therapy scheduling, treatment documentation, productivity monitoring, and outcomes tracking. Maintain accurate treatment records, therapy logs, and care plan documentation within designated systems. Productivity Expectations The Occupational Therapist is expected to effectively manage a daily caseload while maintaining quality clinical outcomes and regulatory compliance. Daily Expectations Treat approximately 10 to 12 residents per day depending on case mix, treatment minutes, evaluations, and resident needs. Complete all required documentation the same day services are rendered whenever possible. Maintain established therapy productivity standards. Coordinate care effectively with

staff, nursing personnel, and interdisciplinary team members. Manage treatment schedules efficiently to maximize resident participation and therapy outcomes. Time Management Expectations The successful candidate will demonstrate the ability to: Balance treatment delivery with documentation requirements. Maintain timely completion of evaluations and progress reports. Prioritize residents based on clinical need and physician orders. Communicate schedule modifications proactively. Minimize missed treatment opportunities. QualificationsRequired Graduate of an accredited Occupational Therapy program. Current Occupational Therapist license in the State of New Mexico. Current CPR certification. Strong clinical assessment and treatment planning skills. Excellent verbal and written communication skills. Ability to work independently and as part of an interdisciplinary team. Preferred Minimum 1 year of experience in a Skilled Nursing Facility, Long-Term Care, Inpatient Rehabilitation, or Post-Acute Care setting. Experience with Medicare Part A, Part B, and Medicaid rehabilitation services. Experience using PointClickCare and NetHealth. Knowledge of PDPM, quality measures, and post-acute care regulations.

Pay:

$50.00 - $60.00 per hour Expected hours: 35.0 - 40.0 per week
