• Per Diem Paradigm Rehabilitation is seeking dedicated Home Care Occupational Therapists to join our multidisciplinary team providing high-quality home-based care to patients throughout the five boroughs of New York City.

This is a per diem, independent contractor opportunity offering true flexibility — set your own schedule with no minimum caseload requirement. Ideal for therapists seeking autonomy, supplemental income, or greater work-life balance while making a meaningful impact. What You'll Do Conduct home visit evaluations for referred patients Assess functional abilities, medical conditions, limitations, and rehabilitation potential Develop individualized treatment plans and provide therapeutic interventions Support pediatric and geriatric patients in improving independence and quality of life Complete EMR documentation (Hello Note experience preferred but not required) Collaborate with families, caregivers, and team members to ensure optimal outcomes Why Join Paradigm Rehabilitation?
