Job Description
What You'll Do:
As an Occupational Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population throughout the Ransomville and Niagara Falls areas. Your caseload may include both home health visits and treatments at our established facility in Ransomville , providing variety while minimizing unnecessary travel.Responsibilities include:
Perform occupational therapy evaluations and develop individualized plans of care. Provide one-on-one treatments based on each patient's functional needs and goals. Manage individualized treatment plans with clinical autonomy. Help patients improve independence, safety, mobility, and their ability to perform daily activities. Complete accurate and timely clinical documentation. Communicate with patients, families, physicians, facility staff, and members of the clinical team. Coordinate with our administrative team regarding scheduling, patient care, and logistics. Manage your caseload based on your availability and patient needs.Compensation:
Part-Time W-2: $50-55 per follow-up visit. $65 per 4-unit evaluation.PRN:
$62 per 4-unit follow-up visit. $75 per evaluation.Benefits:

An active Occupational Therapist (OT) license in New York is required. Reliable transportation required. Strong clinical, communication, and time-management skills. Ability to work independently in home and facility-based settings. Home health experience is preferred but not required.Apply Today:
