What you will do:
Manage a caseload of patients requiring home-based therapy- primarily post acute or subacute- in their homes.
Responsibilities:
Promote/provide Occupational Therapy services in the home to assist the patient in achieving his/her optimal level of functioning. Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions. Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es) Coordinate an interdisciplinary plan of care with n ursing/physical/occupational/speech therapists, medical social workers, aides and dieticians.
What you will need:
NYS Occupational Therapist Licensure 1 year Occupational Therapy experience is required Home care experience preferred but not required Valid Driver's License and Reliable Vehicle. This position provides mileage reimbursement. Be on the front-line of the transitions in health care from inpatient settings to the community - home health care is the future! Eddy VNRA provides a comprehensive orientation program and a supportive work environment including in-depth computer training, assistance obtaining insurance authorizations. Pay Range:
$41.51 - $53.72 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
