Occupational Therapist (OT) - Home Health Summa Health at Home and Hospice |

Akron, Ohio Schedule:

Monday-Friday | 8:15 AM-5:00

You'll be an essential part of an interdisciplinary team dedicated to providing high-quality, compassionate care and achieving positive patient outcomes. In this rewarding home health role, you'll provide skilled occupational therapy services in the patient's home as directed by the attending physician. You'll work closely with patients and their families, helping them regain independence, improve their quality of life, and navigate every step of their recovery journey. You'll educate patients and their families, promote safety and independence, monitor progress, and ensure accurate and timely documentation in accordance with agency, Medicare, and federal requirements. Major Areas of Responsibility Plan of Care Provide skilled occupational therapy as ordered by the attending physician. Observe, document, and report patient reactions, changes, and progress. Develop individualized OT plans of care and adjust treatment as patient needs change. Evaluation Evaluate each patient's level of functioning using appropriate diagnostic and prognostic procedures. Patient Outcomes Use therapeutic activities and home exercise programs to improve range of motion, muscle strength, coordination, mobility, and independence with daily activities. Minimize Patient Risk Help reduce hospital readmissions by educating patients on preventative measures, safety strategies, and healthy decision-making. Family Educator & Advocate Educate and counsel patients and families regarding disease or injury management throughout the course of occupational therapy. Instruct patients and caregivers on the proper use of equipment, including wheelchairs, braces, walkers, crutches, canes, and other prosthetic or orthotic devices. Supervision Oversee and supervise the care provided by Certified Occupational Therapy Assistants (COTAs). Communication & Collaboration Communicate effectively with COTAs regarding supervisory visit findings and patient progress. Collaborate with physicians and other members of the interdisciplinary team. Documentation & Compliance Complete all clinical documentation accurately and in accordance with agency protocols, Medicare requirements, and federal regulations. Policies & Regulations Follow all agency policies, procedures, rules, and regulations. Communicate changes in schedule or availability to schedulers and supervisors. Professional Development Participate in required in-service training and mandatory agency meetings.
