As a key member of our healthcare staff, you will play a vital role in delivering high-quality, patient-centered care to our residents. About the
Role:
We are looking for a motivated and experienced Occupational Therapist to join our day shift team. The Occupational Therapist will work closely with our residents to develop and implement individualized treatment plans that promote independence, mobility, and overall well-being. You will also collaborate with our interdisciplinary team to identify and address any potential barriers to care.
Responsibilities:
Conduct comprehensive evaluations to identify residents' strengths, weaknesses, and goals Develop and implement individualized treatment plans to promote functional independence and achieve residents' goals Provide occupational therapy interventions, including training, education, and coaching to residents and caregivers Collaborate with other team members to develop and implement care plans that meet each resident's unique needs Document resident progress and maintain accurate records in accordance with agency policy and regulations Participate in interdisciplinary team meetings to discuss resident care and develop effective care plans Maintain a safe and organized work environment, adhering to agency policies and regulations
Requirements:
OTR/L certification and licensure in the state of Ohio Bachelor's or Master's degree in Occupational Therapy Minimum 1 year of experience as an Occupational Therapist in a long-term care setting Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment and prioritize tasks accordingly Familiarity with electronic medical records and ability to learn agency-specific software
