Responsibilities include:
Evaluate residents and develop individualized occupational therapy plans of care Provide skilled OT interventions addressing ADLs, functional mobility, strength, safety, and independence Establish appropriate goals and monitor resident progress Complete accurate and timely documentation using Net Health Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team Provide supervision and clinical direction to COTAs as appropriate Educate residents, families, and caregivers regarding treatment plans and recommendations Participate in discharge planning and promote safe transitions Maintain compliance with applicable state, federal, facility, and professional standards Qualifications Graduate of an accredited Occupational Therapy program Current Ohio Occupational Therapist license or eligibility for Ohio licensure New graduates are welcome to apply SNF/LTC experience is a plus, but not required Familiarity with Net Health and PointClickCare (PCC) is a plus Strong communication, clinical, and organizational skills Passion for helping residents achieve their highest level of independence Why Aventura at Humility House?
