Home Health Occupational Therapist

Job Overview Join our dynamic healthcare team as a Home Health Occupational Therapist, where you will play a vital role in enhancing patients' independence and quality of life within their own homes. You will assess individual needs, develop personalized care plans, and deliver targeted therapy to diverse populations, include geriatrics and individuals with disabilities. Duties Conduct comprehensive evaluations of patients' motor skills, sensory processing, and daily living activities to determine therapy needs. Develop and implement tailored occupational therapy care plans that align with each patient's goals and medical requirements. Assist patients with Activities of Daily Living (ADLs), including dressing, grooming, feeding, and mobility to foster independence. Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless transitions across care settings. Provide therapy sessions for individuals across various environments such as home health visits, outpatient clinics, schools, or skilled nursing facilities. Educate patients and their families on adaptive techniques, safety measures, and therapeutic exercises to support ongoing progress. Maintain accurate documentation of patient progress, treatment plans, and case management records in compliance with healthcare regulations. Requirements Valid occupational therapy license with eligibility to practice in the designated region. Proven experience working with diverse populations including geriatrics, individuals with autism or disabilities, and post-acute care patients. Strong knowledge of physiology, anatomy, medical terminology, and functional anatomy relevant to occupational therapy. Experience assisting with ADLs and discharge planning within inpatient, outpatient, home health, or skilled nursing settings. Familiarity with early intervention programs, inpatient care (including ICU experience), outpatient clinics, school-based therapy, and occupational health environments. Ability to work independently in home health settings while managing multiple cases efficiently. Excellent communication skills for collaborating with families, caregivers, medical teams, and educational staff as needed.
