Full-Time Occupational Therapist (OT) - Skilled Nursing & Rehabilitation Uniontown Nursing & Rehabilitation - Uniontown, PA Join Our In-House Therapy Team and Make a Difference Every Day! Uniontown Nursing & Rehabilitation is seeking a compassionate and dedicated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Uniontown, Pennsylvania . This is an excellent opportunity to work in a collaborative Skilled Nursing and Rehabilitation environment where you'll help residents regain independence, improve function, and enhance their quality of life. If you're passionate about providing exceptional, resident-centered care, we'd love to have you on our team! Responsibilities Evaluate residents and develop individualized occupational therapy treatment plans. Provide skilled occupational therapy services to short-term rehabilitation and long-term care residents. Address activities of daily living (ADLs), upper extremity function, cognitive skills, adaptive equipment needs, and functional mobility. Monitor resident progress and modify treatment plans as appropriate. Educate residents, families, and caregivers on therapeutic interventions and safety techniques. Complete accurate and timely documentation using Net Health and PointClickCare (PCC) . Collaborate with physicians, nursing staff, and the interdisciplinary care team to maximize resident outcomes. Maintain compliance with all federal, state, and facility regulations. Qualifications Current Pennsylvania Occupational Therapist (OT) license. Graduate of an accredited Occupational Therapy program. Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply . Experience with Net Health and PointClickCare (PCC) is a plus.
