Educate patients and families on occupational health strategies, safety modifications at home, and adaptive equipment usage. Participate in outpatient clinics, inpatient units, skilled nursing facilities, or home health visits based on program needs. Requirements Valid state licensure or certification as an Occupational Therapist with eligibility for home health practice. Proven experience working with individuals with disabilities across settings such as outpatient clinics, inpatient units, skilled nursing facilities, or home health environments. Strong knowledge of anatomy, physiology, medical terminology, and evidence-based occupational therapy practices. Experience working with pediatric populations including early intervention and school-aged children with disabilities or special education needs. Demonstrated ability to develop treatment plans focused on functional improvements and client-centered goals. Skilled in case management tasks such as discharge planning and coordinating multidisciplinary care teams. Proficiency in electronic health records (EHR) management and clinical documentation standards. Excellent client communication skills to effectively educate patients and caregivers about therapy strategies and safety measures.
