We're building a modern rehabilitation practice that combines: In-Clinic Therapy Home-Based Therapy Telehealth Digital Home Exercise Programs Fitness Integration Long-Term Wellness This is not a traditional outpatient therapy job. We're looking for an Occupational Therapist who wants to help build something innovative while making a lasting impact in the community. What You'll DoEvaluate & Treat Patients Initial evaluations Re-evaluations Individualized treatment plans Functional assessments

training Neurological rehabilitation Orthopedic rehabilitation Balance & fall prevention Fine motor rehabilitation Cognitive rehabilitation Home safety assessments Adaptive equipment recommendations Home Visits Provide occupational therapy services in patients' homes, including: Functional mobility Aging in place Home safety Fall prevention Caregiver education Independence training Telehealth Provide virtual care including: Follow-up visits Home exercise progression Patient education Caregiver coaching Progress reviews Functional reassessments Mobile App Coaching Every patient receives access to our coaching platform. You'll help: Build customized home exercise programs Upload exercise videos Monitor patient progress Review compliance Message patients Perform weekly check-ins Adjust programs remotely Our goal is to continue supporting patients between visits—not just during appointments. Patient Populations You'll work with patients recovering from or managing: Stroke Parkinson's Disease Multiple Sclerosis Balance disorders Falls Orthopedic injuries Joint replacements Arthritis Chronic pain Neurological conditions General deconditioning Functional decline Healthy aging Integrated Care Model You'll collaborate with: Physicians Physician Assistants Fitness Coaches Personal Trainers Nutrition Coaches Recovery Specialists Together, we help patients transition from rehabilitation into long-term health and wellness. Beyond Therapy Unlike traditional therapy clinics, our care doesn't end at discharge.

Qualifications Active Georgia Occupational Therapy License Current CPR Certification Strong documentation skills Excellent communication and patient education Comfortable working independently Positive, team-oriented attitude Comfortable using technology and digital health tools Home health experience is a plus but not required.
