You'll help: Build customized home exercise programs Upload exercise videos Monitor patient progress Review compliance Message patients Perform weekly check-ins Adjust programs remotely Our goal is to continue supporting patients between visits—not just during appointments. Patient Populations You'll work with patients recovering from or managing: Stroke Parkinson's Disease Multiple Sclerosis Balance disorders Falls Orthopedic injuries Joint replacements Arthritis Chronic pain Neurological conditions General deconditioning Functional decline Healthy aging Integrated Care Model You'll collaborate with: Physicians Physician Assistants Fitness Coaches Personal Trainers Nutrition Coaches Recovery Specialists Together, we help patients transition from rehabilitation into long-term health and wellness.
