In this role, you will assess, develop, and implement personalized treatment plans to support individuals of all ages in achieving their functional goals. Responsibilities Conduct thorough occupational therapy assessments to evaluate patients' physical, cognitive, and emotional functioning. Develop individualized treatment plans tailored to each patient's needs, including goals for improving ADLs (Activities of Daily Living) and functional independence. Implement targeted occupational therapy programs across various settings such as inpatient units in our Long term care facilities. Collaborate with interdisciplinary teams including speech therapists, physical therapists, physicians, and educators to ensure comprehensive patient care. Maintain accurate clinical documentation within electronic health records (EHR) systems in compliance with healthcare regulations. Engage in case management activities to monitor progress, adjust treatment plans as necessary, and ensure optimal patient outcomes. Provide guidance on implementing occupational therapy strategies that promote independence in daily activities and workplace safety in occupational health settings. Participate in continuous professional development to stay current with evidence-based practices and advances in occupational therapy, including geriatrics, and physical rehabilitation. Skills Proficiency in conducting occupational therapy assessments and developing effective treatment plans aligned with client needs. Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards. Demonstrated ability to assist clients with ADLs such as dressing, grooming, feeding, and mobility tasks. Familiarity with post-acute care environments including skilled nursing facilities. Adept at managing electronic health records (EHR), documenting progress notes accurately while adhering to confidentiality standards. Excellent client communication skills coupled with the ability to collaborate effectively within interprofessional teams. Knowledge of evidence-based practices for occupational therapy interventions.
