Responsibilities Conduct comprehensive occupational therapy assessments to evaluate patients' physical, cognitive including dementia staging with activities based programs and emotional needs. Develop individualized treatment plans aligned with clinical goals, patient preferences, and evidence-based practices. Implement targeted occupational therapy programs to improve daily living activities (ADLs), motor skills, sensory processing, and functional independence. Assist patients with discharge planning by coordinating care transitions and providing education for continued therapy at home or in community settings. Collaborate with interdisciplinary teams including speech therapists, physicians, nurses, educators, and case managers to optimize patient outcomes. Document clinical progress accurately using electronic health records (EHR) systems while maintaining compliance with medical documentation standards. Support clients with neurodevelopmental disorders such as autism spectrum disorders by designing specialized interventions tailored to their unique needs. Requirements Valid certification as an Occupational Therapist from an accredited program and current licensure in the relevant jurisdiction. Proven experience working with individuals with disabilities across various environments including inpatient and skilled nursing facility Strong knowledge of anatomy, physiology, medical terminology, and evidence-based occupational therapy practices. Demonstrated ability to assist with activities of daily living (IADLS/ADLs), develop treatment plans, and conduct occupational therapy assessments effectively. Familiarity with electronic health records (EHR) management and clinical documentation standards. Excellent client communication skills coupled with the ability to collaborate within multidisciplinary teams for comprehensive care delivery.
